I have read and accept the Facial Contouring Payment Plan — Terms & Conditions, version 1.0 (7 October 2026). I confirm this selection: 13.2 ml facial contouring, total £850, paid in 3 monthly instalments of £283.34, £283.33 and £283.33. I understand that no treatment will be booked or performed until the full £850 is successfully paid and cleared; missed or failed payments may delay or prevent treatment; cancellation and refunds follow the terms above and applicable law; the full paper agreement must be reviewed and signed before treatment; and treatment is subject to consultation, clinical suitability and informed consent.

Facial Contouring Payment Plan — Terms & Conditions Version 1.0 — 7 October 2026 1. This is a pre-payment arrangement for the selected treatment, not a loan, credit or finance agreement. No interest is charged. 2. Instalments are collected through the GoCardless schedule shown for your package. A missed or failed instalment may stop or delay collection. No treatment will be booked or performed until the full package price has been successfully paid and cleared. Part payment does not entitle you to partial or proportional treatment. Cancelling a Direct Debit does not by itself cancel your contractual obligations. 3. Cancellation before treatment: any applicable 14-day cancellation right will be honoured. If no treatment or service has been supplied, refunds will be handled in line with applicable consumer law and these Terms. After the applicable 14-day cancellation period and before treatment, only reasonable, genuine, demonstrable costs or losses actually incurred may be deducted from a refund, capped at £50. The £50 cap is not an automatic fee. Any remaining refund due will be returned. 4. The full paper Payment Plan Agreement must be reviewed and signed at the clinic before treatment. 5. Treatment is subject to consultation, clinical assessment, suitability and informed consent. Payment does not guarantee that treatment will be suitable or that an appointment is available. 6.
